worker canvasses door-to-door in Lackawanna
County making personal contacts with a significant segment of the older
population by providing information on available services and benefits,
assistance in applying for needed services, and assurances that the aging
system is there to lend support and assistance when needed. Telespond's
Outreach worker wears a photo-ID tag identifying
him as an employee of Telespond.
Along with its canvassing efforts, Telespond's Outreach
worker is available to make home visits on request by older adult
consumers seeking emergency assistance, crisis intervention, routine information,
referrals for services, or assistance in completing benefits applications.
Telespond's Outreach program has linked thousands
of older persons to such vital programs and services as home delivered meals,
protective services, domiciliary care, legal services, senior centers, adult
day care programs, entitlement programs such as SSI, Food Stamps, Medical
Assistance, Property Tax/Rent Rebates, PACE, Fuel Rebate & Emergency
Fuel Assistance, as well as the entire range of existing community-based
programs and resources.
Outreach is an integral component of Telespond's
overall mission of service to the older population of Lackawanna County,
Pa., and there is no charge for this service. To arrange for an Outreach
visit, or request additional information, either
e-mail Telespond or phone (570) 961-1950.
This project is funded in part under a contract
with the Pennsylvania Department of Aging and the Lackawanna County Board
of Commissioners through the Lackawanna County Area Agency on Aging.
