PLEASE ... make sure you notify the office of ANY change in address, phone number, doctor or insurance.
If your loved one recently had a change in their prescription(s), suffered an illness or disability, or broken a bone, it is your responsibility as his/her caregiver to notify the Adult Day Services nurse as soon as possible. It is vital that the agency staff have all pertinent and up-to-date information on your loved one so that we may continue to provide the quality care that is such an important part of our mission of service to frail older adults in this community. Everyone attending the Adult Day Services programs has special and individual care needs. In many instances, your loved one is unable to communicate this information to the staff, and we rely on you to do so. Thank you in advance for your cooperation.

Each year we ask our caregivers to PLEASE put the name of your loved one on ALL articles of clothing. You will save us a great deal of time and concern. Due to the number of Adult Day Services participants per day, it is impossible to remember who wore what coat, etc., into the program. Should you have any questions or concerns with labeling your loved one's clothing, please call us at (570) 346-7860.
